HOW TO REGISTER

 

Step 1

  • Download and complete the Registration Form. The fillable version can be typed on, saved to your computer, and e-mailed back to admin@diabetesdayton.org. The printable version can be mailed to Diabetes Dayton (2555 S. Dixie Drive, Suite 112, Dayton OH 45409), scanned and e-mailed to admin@diabetesdayton.org, or faxed to 937-224-0240. 

  • Pay the required registration fee by check (written to "Diabetes Dayton" and mailed to the address above) or paid online via our square account (Click "Make a Payment" in the top right corner of this webpage).

  • This secures a spot for your camper until all spots are filled.

 

Step 2

  • Look for your Camp Application Packet in the mail a few days after your Registration Form & Fee is received.

 

Step 3

  • Submit the Camp Application Packet and Final Payment by the deadline.

  • No exceptions can be made.
