Registration Forms:
Diabetes Dayton utilizes CampDoc for camp registration. http://app.campdoc.com/register/diabetesohio If you are a returning camper, you will use the same login and password as you did in previous years. If you are a new camper, you will need to sign up and then fill out the registration form. Completed registration forms and payment are due before June 19, 2026. Later registrations may be accepted, as bunk availability allows.
Campers are admitted to camp based on having all paperwork completed, and at the discretion of the Medical Director and the Camp Director.
If you are requesting a campership, you are still required to pay the $50 Registration Fee. You will need to complete your request and upload appropriate documents in CampDoc before June 19, 2026.
Physical Form:
Must be completed by your child's PRIMARY care doctor/ pediatrician, NOT the endocrinologist. Please upload the documents to your CampDoc account or have the provider fax the completed form, with attached vaccination record, as needed, to Diabetes Dayton: 937-224-0240
