Registration Forms:
Diabetes Dayton utilizes CampDoc for camp registration. If you are a returning camper, you will be notified when registration opens for the 2024 camp year. If you are a new camper you can fill out the registration form and submit it to our office and you will receive an invitation e-mail from CampDoc to complete your paperwork. Registration forms are due with payment by May 31, 2024. Later registrations may be accepted, as bunk availability allows.
Campers are admitted to camp based on having all paperwork completed, and at the discretion of the Medical Director and the Camp Director.
If you are requesting a campership, you are still required to pay the $50 Registration Fee. You will be able to complete your request and upload appropriate documents in CampDoc before May 31, 2024.
Physical Form:
Must be completed by your child's PRIMARY care doctor/ pediatrician, NOT the endocrinologist. Please upload the documents to your CampDoc account or have the provider fax the completed form, with attached vaccination record, as needed, to Diabetes Dayton: 937-224-0240
